icon_homehealth_servicespg.jpg   Home health care is an efficient and cost-effective alternative to hospital care and other types of institutional care. In some cases home health can provide an earlier discharge from the hospital. Our Home Health team delivers comprehensive medical care within a patient’s home when they are unable to leave it without considerable assistance. We work collaboratively with the patient, the patient’s family and physicians to help the patient recuperate, rehabilitate and learn more about their health. And we help the family stay together in the comfort, security and privacy of the home, wherever that home may be.
     
    Who is eligible for Home Health?
     
   

Everyone is eligible to receive Home Health care regardless of age, sex, race, religion, national origin, handicap, lifestyle, and type of illness or financial situation. However, to receive Home Health services, a patient must meet the following criteria:

  • "Homebound" or unable to leave home without considerable assistance.
  • Under the care of a physician.
  • In need of nursing skills, physical therapy, occupational therapy and/or speech therapy on an intermittent basis.

When being discharged from a hospital or skilled nursing facility, eligible patients must request Central Oregon Home Health and Hospice services by name to avoid being placed with another home health care provider that may not offer the same extensive services or superior quality of care.

     
    The Home Health Care Team.
     
   

Upon admission to COHHH, a team is assigned to the patient and family that will provide care and support throughout their stay in home health and into hospice, in most cases. This team is available 24 hours a day, 7 days a week, every day of the year. A Registered Nurse or a Physical Therapy Case Manager will also be designated to ensure coordinated care. Our Home Health Care Team includes:

  • Nurses

  • Occupational Therapists
  • Speech Language Pathologists
  • Dieticians
  • Medical Social Workers
  • Home Health Aides
  • Chaplains
  • Bereavement Staff
  • Volunteers
