Central Oregon Home Health and Hospice Transitions is a program designed to provide support and assistance to individuals with a life limiting illness without charge.
Transitions is managed by an RN whose goal is to enhance the client’s well-being by:
- Assessing their needs.
- Assisting in navigating through the health care system.
- Empowering their support system.
- Providing referrals and coordination with community and volunteer resources.

Who is eligible for Transitions? |
| |

| |
|
Everyone is eligible to receive Transitions care regardless of age, sex, race, religion, national origin, handicap, lifestyle, and type of illness or financial situation. However, to receive Transitions services, a patient must meet the following criteria:
- Must have a life-limiting illness.
- May be pursuing curative treatment or palliative care.
- Not yet ready to make a decision about choosing hospice care.

All services are provided at no cost wherever the patient resides – their home, nursing home or assisted living facility. |
